Recently we have had insurance companies denying coverage either a sealant or a buccal pit if they are on the same tooth. One plan (state funded) will not pay for a sealant unless it is done on a virgin tooth. So we can either write-off the fee for the sealant, or bring them in for a second visit

A patient wants a copy of the ADA form for an appointment so they can submit it themselves to their employer. Apparently, its a self-funded plan, so there is no insurance company to submit to.
