       What To Do When Plan “A” Does Not Work Out?
In this case, you will see two final restorations. The patient received trauma to the anterior maxillary dentition a number of years ago. #8-10 received RCT. When I met the patient, #8-10 had severe bone loss, blunted roots and class III mobility. The patient knew he needed help with a long-term solution. #8-10 were extracted and two 3i Certain implants placed with the goal of making a screw retained implant restoration. The implants were allowed to osseointegrate for four months before a temporary fixed implant restoration was made. A metal ceramic implant restoration was made and #7 received an eMAX veneer. #7 also had blunted roots and class I mobility. The patient was interested in keeping as many teeth as possible. I thought I could provide an esthetic restoration by keeping #7. I was wrong. The bone between #7-8 was not as robust as I would have like and after healing from stage I, the papilla was gone. I had planned for this by designed a veneer for #7 but we did not get a pleasing result. 
#7 was extracted and a screw retained layered Zirconia fixed implant restoration was designed. Prosthetic gingiva was added to the restoration to simulate the missing papilla. To help aid in oral hygiene, the prosthetic gingiva was not extended all the way to the implant interface. The patient appreciated my efforts to be conservative but being conservative cost me in this situation. Sometimes keeping teeth can create a more challenging situation to restore. These experiences will help when presenting treatment to future patients.
