ADA Draft Guideline Suggests Against Antibiotics for Implants With Augmentation

An ADA guideline panel has recommended against prescribing prophylactic oral antibiotics to healthy adults undergoing dental implant placement with simultaneous site augmentation, in a draft recommendation open for public comment through Aug. 21.

The panel suggests against the use of prophylactic oral antibiotics in healthy individuals aged 18 or older having implants placed with simultaneous site augmentation procedures. It is a conditional recommendation against, based on very low certainty of evidence, rather than a finalized standard of care.

In its rationale, the panel described an uncertain and at most trivial benefit weighed against comparatively better-substantiated moderate-to-large harm, along with equity considerations disfavoring continued prophylactic use and a do-no-harm, patient-centered orientation. The panel reached unanimous agreement on the recommendation.

The scope is narrow. The recommendation addresses healthy adults having implants placed with simultaneous augmentation, and is the first of three sets of recommendations to come from the Living Evidence-Informed Guideline on Antibiotic Prophylaxis for Surgical Procedures Related to Dental Implant Placement.

The ADA is inviting dentists, other health care professionals, patients, organizations, and additional stakeholders to submit feedback. An optional virtual listening session is scheduled for 4 p.m. Central on Aug. 17, though attendance is not required to comment. Comments are submitted through the online form on the ADA’s guideline comments webpage by Aug. 21.

The association said it will review and incorporate public comment into the final recommendation, which is expected to publish this winter.

The guideline is part of the ADA’s ongoing collaboration with the University of Pennsylvania School of Dental Medicine. The ADA established its Living Guideline Program in 2025 to issue evidence-informed recommendations more frequently than traditional guidelines allow, with updates as new evidence emerges. A previous living guideline addressed early oral cancer detection.

The draft remains a draft. The public comment period is the stage at which clinicians who place implants can weigh in before the recommendation is finalized.
