Invisalign With Bite Ramps Produced True Incisor Intrusion in Adult Deep Bites

Clear aligner treatment incorporating precision bite ramps produced measurable true intrusion of the mandibular incisors in adults with deep overbite, rather than correction achieved through tipping alone, according to a retrospective study published Sept. 2 in Scientific Reports.

Investigators analyzed 27 adults between 19 and 54 years old with skeletal Class I or Class II malocclusion and an overbite greater than 3 millimeters, all treated with Invisalign incorporating precision bite ramps.

Lateral cephalometric radiographs taken before and after treatment were superimposed on the cranial base and on the mandible. Mandibular incisor movement was measured using a centroid-based method chosen to isolate pure intrusion and limit the influence of tipping.

The mandibular incisor centroid showed statistically significant pure intrusion of 1.63 millimeters, at P less than .001. Overbite decreased by a mean of 1.98 millimeters, at P = .001.

The occlusal plane angle increased significantly, by a mean of 2.38 degrees at P = .001. Changes in the Frankfort mandibular plane angle, the incisor mandibular plane angle, and mandibular molar extrusion were not statistically significant, which the authors said indicates limited vertical skeletal change during treatment.

The authors concluded that aligner therapy with precision bite ramps demonstrated mandibular incisor intrusion in adult deep-bite patients.

Hyunhee Choo, Young Ho Kim, Kunyoung Chang, and colleagues conducted the study.

The design was retrospective with no control group, and the sample was small and selected. Measurements relied on two-dimensional cephalometric radiographs, which constrains how precisely three-dimensional tooth movement can be characterized.
