Dentists occupy an unusual position. We see the inside of a patient's mouth more regularly than most physicians see the rest of them, and the oral cavity records habits with considerable honesty. Among the patterns worth recognizing is one patients rarely volunteer and almost never connect to what we are looking at. Their regular evening drink.
What Shows Up in the ChairErosion is often the first clue. Wine, ciders, and cocktails built on citrus or carbonated mixers are acidic, and nightly exposure produces the smooth, cupped erosive patterns we typically associate with reflux or citrus habits. When a patient with clean brushing and reasonable diet presents with facial and incisal erosion that does not match their reported intake of soda or fruit, the evening glass of white wine is worth asking about.
Reduced salivary flow is the second finding, and arguably the more consequential one. Alcohol is a diuretic and contributes to xerostomia, which removes the buffering, remineralizing, and clearance functions saliva provides. The downstream effects are familiar. More plaque accumulation, higher caries risk, particularly cervical and root surface lesions in older patients, and increased sensitivity. Add sugary mixers or sweet wines and the caries picture worsens further.
Periodontal findings also correlate. Patients who drink regularly tend to present with more inflammation and deeper probing depths than their home care would predict, and the tissue response to therapy can be less predictable than expected.
Surgical and Healing ConsiderationsFor practices doing extractions, grafting, or implant placement, intake matters to outcomes. Alcohol affects platelet function, hydration status, and tissue healing, and it complicates the postoperative period when patients continue a nightly routine they never thought to mention because nobody specifically asked. Postoperative instructions that say to avoid alcohol are easy for a patient to interpret loosely if the drink is so habitual that it barely registers as a decision.
The same is true of medication interactions and, for many patients, of sleep. Fragmented sleep from evening drinking is worth noting in patients presenting with bruxism, myofascial pain, or apnea symptoms, where the interaction can amplify what we are trying to manage.
The Screening ConversationAlcohol is also a recognized risk factor in oral cancer, with a synergistic relationship with tobacco. That makes an accurate intake history relevant to how we frame our screening interval and what we tell patients about their own risk profile. Yet intake histories are notoriously unreliable, and not because patients intend to mislead us. A generous home pour of wine can be closer to two standard drinks than one, so a patient reporting two glasses may be describing something closer to four by clinical measures.
How to Raise It Without Losing the RoomThe clinical challenge is less about detection than delivery. Approach the subject with any hint of judgment and most patients close the conversation immediately, which serves nobody. The research on behavior change is consistent on this point. Shame produces concealment and defensiveness, while curiosity keeps attention open, and change driven by a patient's own reasoning holds far better than change imposed by instruction.
Practically, that means framing the question around what you are seeing rather than around the patient's character. Describing the erosion pattern and the dry mouth, then asking what their evenings typically look like, tends to produce more useful answers than a direct inquiry that sounds like an accusation. It also helps to explain the mechanism. Patients are frequently unaware that a nightly glass of wine has anything to do with their teeth, and the connection alone can motivate change without any prompting from us.
Where to Point ThemWhen a patient decides they want to cut back, most do not need a program. They need a way to make an automatic routine visible. Most drinking habits are cued rather than chosen, triggered by the end of the day and repeated with little conscious involvement, which is why advice to use more willpower rarely accomplishes much.
Simple tracking helps, and it doubles as better history for our records. Some patients respond well to gentler techniques such as alcohol hypnotherapy, which uses guided sessions to loosen the automatic quality of a long standing routine.
