This young healthy patient broke off the lingual half of #4 down into the gums about 3mm. Very painful. Although clearly no pulpal involvement. Clearly sufficient tooth structure above the gum line but so deep on the lingual. How do you handle these? Why is he in pain if no pulpal involvement

and hold it against the teeth, then make marks at the location of all of the incisal edges with a pen/marker/whatever. This works well. Punch the central incisors 1 inch from the edge of the dam. And you can also sometimes place a 2A clamp right over the dam onto a 2nd bicuspid, without any punch. It will be more comfortable for the patient with a layer of rubber dam covering the tooth and gum, but doesn't hold as well as with a punch.

curious, the troughing looked perfect.Only because I seem to have a harder time getting the job done with pulse on healthy keratinized tissue..........I really only use pulsed on areas where the patient isn't numb and that's only if they are feeling it after I've tried with continuous first. I have a similiar concern.Perhaps Marginal elevation with amalgam as the restorative material works better than composite.

for treatment. For a healthy adult I'm very happy to place an occlusal amalgam. It has plenty of silver in it, and doesn't need to be reapplied. When it arrests the caries lesion, it turns the tooth tissue black. There is one brand that includes an application of Potassium Iodide after the SDF, which forms a white precipitate: it is called Riva Star. It is still not advisable to use it in the aesthetic zone though.
