Flaps and Sutures: Cracking the Code

by Dr. Giacomo Tarquini

This comprehensive CE online course by Giacomo Tarquini focuses on flap design, soft tissue management, and suturing techniques in implant and regenerative surgery, with particular emphasis on achieving predictable, tension-free wound closure. Through a biologically driven and clinically oriented approach, the course analyzes how incision design, flap elevation, tissue handling, and suturing directly influence vascularization, healing stability, membrane exposure, and long-term surgical success.

Participants are guided through the principles of crestal, paracrestal, vertical-releasing, intrasulcular, and paramarginal incisions, as well as full-thickness, split-thickness, and mixed flap approaches. Advanced flap passivation techniques—including Rehrmann plasty, submucosal space preparation, and superficial-layer split-thickness flaps—are presented in detail for guided bone regeneration procedures in both the maxilla and posterior mandible. Special attention is dedicated to the management of critical anatomical structures such as the mental nerve, mylohyoid muscle, lingual tissues, and mandibular concavities.

The course also provides an in-depth review of suturing objectives and techniques in implant surgery and GBR, including interrupted, continuous, mattress, suspended, and periosteal sutures, with evidence-based recommendations regarding flap stabilization and ischemia prevention. Finally, multiple surgical protocols for the treatment of oro-antral communications and fistulas are discussed, including palatal rotational flaps, buccal fat pad grafts, and combined approaches, supported by clinical cases and extensive literature references.

Upon completion of this course, you should be able to:
  • Gain a clear and practical understanding of periodontal disease, confidently distinguishing periodontal health, gingivitis, and periodontitis using up-to-date diagnostic criteria.

  • Understand the logic and clinical value of the 2017 AAP/EFP classification, moving beyond outdated labels to a modern, biologically driven framework.

  • Confidently apply staging and grading in everyday practice, transforming a system often perceived as complex into a powerful tool for diagnosis, treatment planning, and prognosis.

  • Recognize and assess the key local and systemic risk factors that influence disease progression and long-term outcomes.

  • Adopt a structured, efficient periodontal diagnostic workflow, integrating clinical examination, periodontal charting, and radiographic analysis to support accurate, defensible clinical decisions.

Giacomo Tarquini, DDS, graduated with honors in Dentistry and Dental Prosthetics from the University La Sapienza of Rome in 1994. He completed his training and specialized in periodontology at the Department of Periodontology of University La Sapienza of Rome. Today he practices the free profession in Rome with particular interest in the disciplines of periodontology and implantology. He is also a consultant, professor, tutor and lecturer for a variety of dental specialties. Along with various articles, Tarquini is the author of the textbook, Techniques of Periodontal Surgery from Diagnosis to Therapy.
