As-Needed Ibuprofen and Acetaminophen Matched Scheduled Dosing After Emergency Endo

Patients who took ibuprofen and acetaminophen only as needed after emergency endodontic treatment reported pain levels similar to those of patients who took the medications on a fixed schedule, while using about half as much medication, according to a randomized controlled trial published online Oct. 21 in the Journal of Endodontics.

The trial enrolled 172 healthy patients who came to Ohio State University’s graduate endodontic clinic for emergency treatment of a painful tooth diagnosed with symptomatic apical periodontitis. Of those, 126 completed and returned diaries and remaining pills documenting their pain and medication use for three days after treatment: 58 assigned to scheduled dosing and 68 assigned to on-demand dosing.

Patients in the scheduled group took significantly more medication (P < .05), roughly twice as much as patients told to medicate as needed. Pain scores did not differ significantly between the groups.

The authors noted that patients with symptomatic apical periodontitis face a higher risk of significant postoperative discomfort. They also noted that although the ibuprofen-acetaminophen combination is widely used, little evidence has compared scheduled and on-demand dosing in this population.

They concluded that on-demand dosing appears sufficient to manage postoperative pain in these patients and could reduce medication use and associated side effects.

Madeline Malueg, Jaclyn Danko, Melissa Drum, and colleagues at Ohio State University conducted the study.

More than a quarter of enrolled patients did not return complete diaries, and the trial ran at a single university clinic. The published abstract does not specify the doses used in either regimen.
