Synopsis:
Achieving consistent success with tissue-supported complete dentures requires well-executed laboratory procedures and a structured clinical evaluation at the try-in acceptance appointment. This course guides readers through key steps in posterior tooth arrangement using lingual control lines and establishing lingualized occlusion. It also presents a practical remount technique for correcting mounting inaccuracies. To ensure accuracy, the case must be returned from the laboratory mounted on the articulator, allowing the dentist to verify the correctness of the mounting. Remounting with a new centric relation record is discussed as a necessary corrective step when mounting errors are identified. For predictability, it is essential to verify that mounting is accurate.

Educational Objectives:
Upon completion of this course, you should be able to:
  • Describe important laboratory procedures for denture fabrication, including using lingual control lines for positioning posterior teeth and establishing lingualized occlusion.
  • Perform a clinical evaluation of the wax try-in setup, make appropriate modifications, and approve the setup before presenting it to the patient.
  • Explain the importance of assessing mounting accuracy by comparing the occlusion on the articulator with the intraoral occlusion.
  • Identify when a clinical remount is indicated to correct mounting errors and describe how to record centric relation for the remount.
  • Outline the recommended process for guiding patient evaluation and obtaining approval of the denture setup during the clinical try-in acceptance appointment.

Thank you Dr.Stromberg ,another great lecture. Lab instructions again v important for try in appointment (keep the same articulator), along with pictures and patients descriptions of desired teeth, Lingual control lines ( remember drawing lines on the residual ridges as a student but did not know those are arbitrary and fallacious) , and lingualized occlusion for set up. The more lingual the lower teeth is the more stable is the denture. Do you recommend any specific base plate for better retention on the maxilla or mostly use adhesive to get through the try in appointment? I am so glad to hear you do not use wash impression at the try in appointment (i learnt this while working in a corporate office though never preferred to do it. Advantages and disadvantages of different degrees of cusp in denture occlusion. Anatomic vs semi anatomic vs non anatomic. Is there a way to confirm if the lab has used the teeth specifics as requested by the dentist (do they provide some sort of stickers of the teeth, it would be helpful if someone hasn't done a lot of dentures to be able to quick to identify if any errors in selection by the labs). Use of bite material that has some resistance and is not runny like a PVS, Rolling the tongue can retrude the mandible but not necessarily position the jaw in CR. Observe upper denture as pt closes for remount cases, if any movement of the upper denture during closure, repeat the records. Tips of wall mirror vs. hand mirror, use of cool spray to solidify wax if any alterations are done. Overall, great CE, highly recommend to friends and professionals.
