Dr. Barry Buckley runs Clearbraces.ie, which has initiated 4,000 Invisalign cases, making it the number one Invisalign account in Europe for three years running.
Buckley provides both clinical and management support for a network of dentists, who are all GDPs and had no experience in orthodontics before they worked with him. On average, each now achieves more than 100 cases per year.
This 26-year-old patient attended for a cosmetic consultation. Her complaint: ‘I don’t like the colour of my front teeth and I want to straighten my teeth’.

Extraoral examination: Skeletal Class III base, with an increased Frankfort mandibular plan angle (FMPA) and an increased lower anterior face height (LAFH).
Intraoral examination: Class I incisor relationship with retroclined lower incisors. Reduced overjet and reduced overbite, and mild upper and lower crowding.
Upper left 6 (26) and upper right 2 (12) were in crossbite, with no displacement.
The molar relationship was Class I on the right side and 1⁄4 unit Class III on the left side.
The patient had a heavily restored dentition.
There was severe staining/mottling of the upper anterior teeth.
Orthodontic treatment
After treatment options were discussed with the patient, she was happy to begin the orthodontic phase to improve the occlusion and align the dentition.
A 16-month Invisalign treatment plan to align the upper and lower arches and make space in the overjet for the restorative phase of treatment was completed.

Upper and lower impressions and a bite registration were taken for the dental technician to construct a diagnostic wax-up.
The wax-up of the planned end result was shown to the patient. She was happy to proceed with the removal of the bridge replacing the upper right 4 (14) and preparation of upper right 2,1 (12, 11) and upper left 1,2,3 (21, 22, 23).
