Meta-Analysis Links Oral-Health Burden to 31% Higher Cardiovascular Risk

A systematic review and meta-analysis of 30 cohort studies has found that periodontal or broader oral-health problems are associated with a 31% higher risk of developing cardiovascular disease. The analysis, published July 3, 2026, in BMC Oral Health, drew on adults who were initially free of cardiovascular disease.

The pooled estimate produced a hazard ratio of 1.31, with a 95% confidence interval of 1.13 to 1.48. That main figure reflects a broad periodontal and oral-health exposure; the authors ran restricted sensitivity analyses for clinically diagnosed or periodontitis-coded exposure and for confirmed major cardiovascular outcomes. The association was strongest for stroke and showed a dose-response pattern, with greater disease severity corresponding to higher risk.

The analysis also found that periodontal treatment was correlated with a reduction in cardiovascular risk. The authors appraised study quality with the ROBINS-E tool and searched the literature from 2000 to 2024, following PRISMA guidelines, with the protocol registered in PROSPERO.

They were careful about the limits. Significant heterogeneity, driven largely by differences in study design and in how oral-health exposure was defined, constrains how firmly the pooled estimate can be interpreted. Because the underlying studies are observational, the analysis does not establish that periodontal disease causes cardiovascular events or that treating it prevents them.

The authors position oral-health management as a component of comprehensive cardiovascular risk mitigation rather than a proven preventive intervention. The review was led by M.K. Alam and colleagues, with authors across institutions in Saudi Arabia, Syria, and the United Arab Emirates.
