One decision.

Half the patients gone almost overnight.


And an entire community talking about it on Facebook.


From the outside, buying a practice looked like a huge step forward.


What most people didn't see was what happened next.


After Delaney Spaulding purchased the practice, one of the first decisions made was to go out of network with all insurance plans. Patients could still use insurance, but the practice would no longer be a preferred provider.


The result?


Nearly 50% of the patient base disappeared.


The schedule became alarmingly empty.


And public criticism showed up everywhere.


What looked like a business decision quickly became a leadership test.


Here's what I learned:


1. The hardest part of leadership is making decisions before there is evidence they will work.


Many business owners want certainty before they act. But some of the most important decisions only reveal their value years later. In the moment, they often feel like mistakes.


2. A business strategy becomes real when it forces better execution.


Without insurance driving patient decisions, the practice had to become intentionally excellent at communication, patient experience, and trust. The pressure exposed what needed to improve.


3. Not every complaint is a sign to change course.


Sometimes criticism is valuable feedback. Other times it's simply the cost of choosing a direction. Leaders must learn the difference.
