  • Did good radiographic technique occur
         
  •     
  • Was a rubber dam used
         
  •     
  • Why did a tooth fracture after treatment
         
  •     
  • Was there broken or separated instruments
         
  •     
  • Were proper root canal instruments used
         
  •     
  • Did a Hypochlorite accident occur
         
  •     
  • Obturation problems: the question of underfilling and overextension versus overfilling of the root canal system
         
  •     
  • Perforations
