My patient presented with no maxillary restoration and a mandibular one that was in poor condition. Before we even thought about implants, I made a conventional denture and two mandibular FDPs (bridges), which helped me establish a plane of occlusion, tooth position based on esthetics and phonetics and determine restorative space. I was then able to measure and see how much room I had for restorative parts using a silicone matrix of the tooth position. I failed to achieve adequate retention with the conventional restoration due to a shallow vestibule and short ridge height. I had 15mm + of restorative space, which I was able to use to design a palate-less bar retained and supported over-denture with three Locator Attachments. My team and I were only able to get three implants in the patient's available bone. Patient declined grafting procedures. I duplicated the first denture in clear acrylic and that served as a surgical guide. I prefer to splint the implant impression copings together when I am making a multi-implant impression. I used a like cure material called TRIAD gel in this instance. The design provided ample retention for the patient and he was very satisfied with the outcome!
