Dr. Brent graduated from Tufts University School of Dental Medicine in 1988. Prior to Dental School, he attended SUNY Albany, graduating in 1984 with a BA in Biology and a minor in English Literature. Dr. Brent has been practicing for over 30 years. He has been in his current location for 11 years. Since he began practicing dentistry, Dr. Brent has kept up to date by attending many continuing educational classes, ensuring that his patients receive the most comprehensive and advanced dental care. Dr. Brent has lived and worked in Middlesex County for over 20 years.

Howard Farran: Hey, I'm at the Greater New York Dental Meeting, and I ran into a buddy of mine, Larry Brent, who's been a dentist from the top since what? '80 ... 

Larry Brent: '88.

Howard Farran: ... '88, and you have something really interesting and it's a different type of anesthesia. The reason that I brought him over here to talk about it is, Dentists always get together, and they always tell me the importance of same-day crowns, and then that that's just, like, the biggest movement in dentistry. All these patients want same-day crowns; and I don't know what you're talking about, because I've been a dentist for 30 years, and when I tell a patient their tooth's broken, half the questions are, "Well, how much is it going to cost?" and "Will my insurance pay?" and "Can I make payments?" and "Care Credit," and all that.

Then, when we get done with that, then it switches to pain: "Well, am I going to get a shot?" "Is it going to hurt?" "Can you give me laughing gas?" "Can you put me to sleep," blahblahblah ... and then it's, "Well, what is it going to look like?" "Is it going to look blahblahblah ... "

... and the only thing that dentist heard was, "Well, I want it in the same day." Is it like that, because that's the only thing I never even did hear; and that's all I really thought about, and when I walk by you, and you got a different mouse trap for the age-old problem, and so I don't even know about it. Tell, tell us all about it! It's Numbee.

Larry Brent: Okay, so the product is called Numbee, and it is basically a single tooth anesthesia system. The unit, basically, as you'll see here is a small cannula that will connect to a ligamentary-style syringe or to any traditional syringe that you have in your own office. The interesting thing about this product is there is no needle, Howard. You can feel the end there, there's nothing that will poke you, there's nothing that will hurt you. You can let a kid do that in the operatory.

The basic principle that this works by is that the plastic cannula goes into the gingival sulcus, you seed it to the base of the periodontal ligament and then draw back a little bit, and begin slowly to let anesthetic seep into the periodontal ligament. The tissue will blanch, generally it takes about 2 to 3 clicks per root of a tooth in order to numb a tooth. It is phenomenal for Pediatrics, I have used it for extractions, I've used it for crown preparations, I've used it for endodontics. Patients do not require a topical anesthetic and there is no residual external numbness involved when your procedure is completed.

Howard Farran: Okay, if you're listening to this on iTunes, you might want to switch to YouTube or Dentaltown to see it but ... It looks kind of like a ligament jet.

Larry Brent: It's like a ligament jet.

Howard Farran: What is the difference between this and a ligament jet?

Larry Brent: The difference is that a ligament jet has a needle at the end, and generally, I was taught that you need to pierce the periodontal ligament at the base of the sulcus and inject anesthesia, under very high pressure, into the periodontal ligament in order to accomplish numbness. I used the ligament jet in my practice as the last resort anesthetic. I've given a block, I've given an infiltration I can't get the tooth profoundly numb then I used to go to that. What I would find, when that worked, and it worked most of the time for me, with the patients having more complaints about the postoperative discomfort of piercing that ligament and breaking the bond of the sulcus. I've had no postoperative discomfort or reports of discomfort following this.

Howard Farran: Well, some of the resource I read on ligament jets said that they didn't even need anesthetic. It was the pressure. You were injecting pressure water, if you just injected water into the periodontal ligament as the pressure built on the ligament, that would be anesthesia too; so it wasn't even the anesthetic, it was the pressure.

Larry Brent: Interesting. I did not, I have not read that.

Howard Farran: That's what I read; but the problem with being 53 is that some of the stuff I learned was in '87 ... 

Larry Brent: Yes.

Howard Farran: ... so, we still, us, old guys know a lot of stuff that's no longer true.

Larry Brent: We think the old way.

Howard Farran: Yeah; so then, you're not injecting pressure into the ligament like a ligament jet. You're just-

Larry Brent: Not under ... it's not under tremendous pressure you can't really break the cartridge.

Howard Farran: You're just depositing anesthetic-

Larry Brent: This [product 00:03:47] is depositing anesthetic under mild pressure.

Howard Farran: Who invented this?

Larry Brent: This product was invented by ...

Speaker 3: By a founder who remained anonymous, on the West Coast. California.

Larry Brent: ... but a practicing dentist.

Speaker 3: Absolutely.

Howard Farran: Why does he want to be anonymous?

Speaker 3: Just doesn't want his name in the spotlight.

Howard Farran: Yeah, and that's true of the lot of the old dentists. I remember when I was young and I was trying to do interviews, like Bob Ibsen, the founder of Den-Mat, he didn't want to be interviewed, Dan Fisch of [Ultradent 00:04:15], Bill Dragan of Centrix Syringe ... Every time I say, "Well, I want interview them, everyone wants to know the dentists behind the companies," he was like, "No, no, no." They all seem to be shy introvert, don't want a lot of attention.

What percent of the time are you using that for anesthetic?

Larry Brent: I'm using this about 85 to 90% of the time now.

Howard Farran: For what procedures?

Larry Brent: All procedures.

Howard Farran: I mean a root canal?

Larry Brent: Yes.

Howard Farran: Root canal, extraction?

Larry Brent: Yeah, root canal, extraction.

Howard Farran: Are you serious? ...

Larry Brent: Crown preparation ...

Howard Farran: ... that you're using that for an extraction.

Larry Brent: Yeah. Pediatric and adult extraction.

Howard Farran: Wow; and so the name of the company is Numbee.

Larry Brent: The name of the company is Novoject, N-O-V-O-J-E-C-T, .com ... 

Howard Farran: For Novocain, I'd say.

Larry Brent: ... and the name of the product is the Numbee.

Howard Farran: Where did ... so Novoject came from Novocain injection.

Larry Brent: Right.

Howard Farran: Where did Numbee come from?

Speaker 3: Numbing from a sting.

Howard Farran: Numbing from a sting.

Speaker 3: Instead of getting stung, you get numb instead.

Howard Farran: Instead of getting stung from a bumblebee, you're getting numb from a bee.

Speaker 3: Exactly.

Howard Farran: That's cute. I like it. Like I say, I think a lot of older dentists have told me, in fact a dentist told me just yesterday just right here, he said, "You know what?" He was 65, and he's retiring. I said, "We'll stop by and just talk to the young kids, you're getting ready to retire," and most of the people watching these podcasts, they've all been out of school under 5 years, and he said, "You know what? If you can't give a painless shot, you're not going anywhere." I mean ... 

Larry Brent: Got to be Painless Paul.

Howard Farran: You got to be affordable, available, and painless. The dentists all want to hear that you need a laser, and a CAD/CAM, and a same-day crown; and, you know, they just invent stuff. They just spend all their time, their whole careers, figuring out how to do dentistry more complicated, more expensive with higher [inaudible 00:06:00] red, and I'll tell you: At the bottom of the day, be affordable, be available, and have a pain-free injection. That's what you're saying.

Larry Brent: That's exactly what this does.

Howard Farran: How much more pain free do you think that is than a block or a shot or a ... 

Larry Brent: Significantly, and I pride myself on being Painless Paul and giving you a good injection.

Howard Farran: Painless Paul; and then go to [what 00:06:16] the cost is. How much would be the syringe, how much would be the ... 

Larry Brent: The syringe runs ...
